Transition IPP
You will receive a transition IPP moving you into the self-determination program. It is different at each regional center when this occurs.
Some regional centers do an IPP at the beginning of the process during budget and just send a copy of the IPP to sign when all of the paperwork is complete. Others will have a meeting at the end of the process. Some do not have the IPP done until after the transition.
It is important that your goals that relate to items in your spending plan are in the IPP. Do a final check to make sure all the goals from your person centered plan you wanted included are there.
Set Up Employees/Services
Background Checks
All staff who provide direct personal care to SDP participants must pass a background check. The FMS will provide a form for staff to take to a registered fingerprinting center. Each Financial Management Service has its own process, so check to see how yours works. Results take 3–14 days. If a staff member has a minor crime, they can ask for special permission to work. If they have serious crimes they cannot be hired. Staff who worked in SDP before with another FMS do not need a new check, but those from other programs will.
Verify Purchase of Service have been sent and received
The last step is following up with both your service coordinator and the FMS that the Purchase of Service (POS) from your IPP have been sent to the FMS. Check with the FMS that you are all set up and ready to go!
Some FMS have a requirement that the POS are in their system prior to the start date. Check with your FMS to find out when they need the POS.
