Enrollment Requirements:  Referral form to be completed by Regional Center, draft spending plan must be sent to the fms@sentinelfour.com email.

Start Date Requirements: They must have a signed and approved spending plan by the 10th of the month prior to start date. Authorizations must be in place with the Regional Center by the week prior to the start date.
